We will do this by creating opportunities for people to build on their existing abilities, empowering people to make choices and live the life they choose, and through fostering participation and inclusion in their community.

Supported Community Access and involvement

Feeling connected to your community is important to your happiness and sense of wellbeing.

Supporting Abilities will support you in taking part in activities that you enjoy, giving you the opportunity to meet and mix with people who share your interests. Some ways that Supporting Abilities might help with are outings to the cinema, park or museum, perhaps playing a sport, shopping, joining a club or going on holiday.

We will find and match you with one of our Direct Support Professionals based on your shared interests. Our aim is to support you to achieve your unique goals and aspirations and to help increase your sense of independence as a valued and participating member of the community. Your Supporting Abilities support professionals will accompany you to activities, whether it’s shopping, playing sport, or just hanging out in a café.

At Supporting Abilities, we provide a quality lifestyle and personal support service by responding to the individual needs of people living with a disability creating choices and helping them to achieve their potential for independence.

Supporting Abilities Support Coordination strengthens and enhances participants lives.

Our Support Coordinators work promptly, creatively and resourcefully with participants to ensure their needs and goals are attained through a variety services available in their community.

Support Coordinators will assist participants to negotiate services and prices for supports and will assist in building capacity for participants to implement their supports.

The Supporting Abilities team can support you by:

Talking through any difficulties you are experiencing in getting the support you need

Specialised Support Coordination

This is a higher level of support coordination. It is for people whose situations are more complex and who need specialist support.

A specialist Support Coordinator will assist you to manage challenges in your support environment and ensuring consistent delivery of service.

Depending on your individual goals, plan objectives and aspirations you may receive funding for these supports in your plan.
